Histamine and estrogen: how they feed each other

Histamine and estrogen push each other up. In lab studies estradiol makes mast cells release what they store, and histamine makes ovarian cells release more estradiol.

That loop is real in the dish. Most of what the internet builds on top of it, for anxiety, ADHD, perimenopause and PMDD, has much less behind it. Here is what I found.

Histamine and estrogen: what the studies show

  • Estrogen switches on mast cells. Mast cells store histamine. In mast cell lines and mouse mast cells, estradiol at physiological concentrations triggered a partial release of their granules and strengthened the response to allergens. It worked through estrogen receptor alpha and a rise in calcium inside the cell. (Zaitsu 2007)
  • Testosterone did the opposite. In purified rat mast cells, estradiol increased the secretion of histamine and serotonin. Testosterone inhibited secretion. The estrogen blocker tamoxifen also inhibited it. (Vliagoftis 1992)
  • Histamine raises estrogen output. Ovarian granulosa cells taken from 17 women released more estradiol as the histamine dose went up. An H1 antihistamine blocked the effect. (Bódis 1993)
  • The enzyme that clears histamine moves with the cycle. In 36 healthy women aged 20 to 29, blood levels of diamine oxidase (DAO) were significantly higher in the luteal phase than in the follicular phase. (Hamada 2013)

This fits how I see both of them. Estrogen and histamine are stress signals that travel together, with serotonin close behind.

Histamine and perimenopause

A 2026 review says that declining and fluctuating estrogen and progesterone change mast cell activity, and that menopause may worsen allergic disease or trigger it for the first time. The same review says the mechanisms are poorly understood and evidence-based guidelines are scarce. (Valerieva 2026)

So the idea is taken seriously. I did not find a trial that treated perimenopausal symptoms by lowering histamine.

Histamine and PMDD

I searched PubMed for histamine and premenstrual dysphoric disorder and found no modern trial. The only cycle data I can give you is the DAO study above, and it shows more of the clearing enzyme before the period, not less.

So if someone tells you PMDD is a histamine problem, ask for the study. PMDD is a medical diagnosis and it deserves a doctor.

Histamine and anxiety

The human evidence comes from a drug. Hydroxyzine is a prescription antihistamine. A Cochrane review of 5 trials with 884 participants found it more effective than placebo for generalised anxiety disorder. The reviewers still would not recommend it as a reliable first choice, because the trials were few, small and at high risk of bias. It was also associated with more drowsiness than the comparison drugs. (Guaiana 2010)

That tells you a sedating antihistamine can calm anxiety. It does not prove that high histamine causes anxiety.

I have posted a lot about cyproheptadine, which blocks histamine and serotonin. It is a prescription drug. Talk to a doctor before you go near it.

Histamine and ADHD

Three studies, and they do not point the same way.

  • Food additives. In a placebo-controlled crossover trial in 153 three-year-olds and 144 children aged 8 to 9, food colours and a preservative worsened ADHD symptoms more in children with certain variants of HNMT, a gene for an enzyme that breaks down histamine. (Stevenson 2010)
  • Antihistamines. In 42 children with eczema and no ADHD diagnosis, past antihistamine use was associated with more ADHD symptoms, with an odds ratio of 1.88. The authors could not tell whether the drug or the severity of the eczema was responsible. (Schmitt 2018)
  • A histamine drug for ADHD failed. 430 adults were randomized to bavisant, which acts on the brain's H3 histamine receptor, or to placebo or standard drugs. At the highest dose the symptom score fell by 12.2 points against 8.8 on placebo, which was not significant. Atomoxetine and methylphenidate both beat placebo. (Weisler 2012)

Histamine in the brain keeps you awake and alert. So blocking it is not an obvious win for attention.

Histamine foods to avoid

The medical term is histamine intolerance: more histamine coming in than the gut enzyme DAO can break down. (Maintz 2007)

The foods named in the reviews are the aged and fermented ones:

  • Scombroid-type fish, the tuna and mackerel family. This is the classic source of histamine poisoning.
  • Cheese.
  • Wine and other alcohol.
  • Dry sausage.
  • Sauerkraut.
  • Miso and soy sauce.

That list is from a food science review. (Stratton 1991) Alcohol and some drugs can also release histamine or block DAO.

  • Does the diet help? 18 patients with histamine intolerance followed a low-histamine diet and a normal diet in random order. On the low-histamine diet they reported significantly fewer gut and skin symptoms. Their DAO blood levels did not track with the diet. (Rentzos 2024)
  • DAO capsules. 28 patients took DAO before meals for 4 weeks. All 22 symptoms on the questionnaire improved, and they came back after stopping. There was no placebo group. (Schnedl 2019)

I eat fresh meat, fresh milk, fruit and well-cooked food, so most of this list is not on my plate anyway.

The honest part

  • The estrogen and mast cell studies are in cell lines and rats. Nobody has shown the loop running in a living woman.
  • A 2020 review says more scientific evidence is still needed to define, diagnose and manage histamine intolerance. (Comas-Basté 2020)
  • The diet studies had 18 and 28 patients.
  • For PMDD I found nothing, and for perimenopause only a review.
  • Hives, swelling of the lips or throat, or trouble breathing are an emergency, not a diet question.

What I do

My main tool is carbon dioxide. In rat mast cells, pretreatment with 100% CO2 inhibited degranulation and histamine release by more than 95%, by stopping the rise in calcium inside the cell. (Strider 2011)

That experiment used pure gas on cells in a dish. My step from there is my own view: a body that burns carbohydrate well makes more CO2, and I think that keeps mast cells quieter. So I eat high carb, I do not fast, and I keep salt and calcium up.

Quick answers

What is the link between histamine and estrogen?

In lab studies they stimulate each other. Estradiol made mast cells release their contents through estrogen receptor alpha, and histamine made human ovarian granulosa cells secrete more estradiol through the H1 receptor. This has not been confirmed in living people.

Does histamine cause anxiety?

It is not proven. The prescription antihistamine hydroxyzine beat placebo for generalised anxiety in a Cochrane review of 5 trials, but the trials were small and at high risk of bias, and a drug effect does not prove the cause.

Is histamine linked to ADHD?

The evidence is mixed. Children with certain histamine-degrading gene variants reacted more to food additives, past antihistamine use was associated with more ADHD symptoms in children with eczema, and a histamine H3 drug failed in a trial of 430 adults.

Does histamine get worse in perimenopause?

A 2026 review says fluctuating estrogen and progesterone alter mast cell activity and that menopause may worsen or trigger allergic disease, but it also says the mechanisms are poorly understood. No treatment trial was found.

Is PMDD caused by histamine?

No study found in PubMed shows that. One study found the histamine-clearing enzyme DAO is higher, not lower, in the luteal phase of the cycle.

What histamine foods should you avoid?

The foods named in reviews are aged and fermented: scombroid-type fish such as tuna and mackerel, cheese, wine, dry sausage, sauerkraut, miso and soy sauce. In 18 patients a low-histamine diet reduced gut and skin symptoms.
