BPC-157 vs TB-500: What Each One Is, and How Little Human Data TB-500 Has

BPC-157 and TB-500 are sold together as a healing stack. They are different peptides from different parts of the body, and the only study that tested them together found no extra benefit from combining them.
TB-500 has even less human evidence than BPC-157. I could not find a single human trial of TB-500 itself.
What is TB-500?
Thymosin beta-4 is a natural peptide of 43 amino acids that helps cells move, and it promotes blood vessel growth and wound healing. (Philp 2004)
TB-500 is not that molecule. When chemists analysed a product sold as TB-500, they found a 7 amino acid piece of it, positions 17 to 23, with an acetyl group added to the front. (Esposito 2012)
That short segment is the region of thymosin beta-4 that binds actin. TB-500 appeared as a veterinary preparation, and the early papers on it come from doping-control laboratories, including one building a test to catch it in racehorses. (Ho 2012)
So when a site quotes thymosin beta-4 research to sell TB-500, it is borrowing the evidence of a larger molecule.
BPC 157 vs TB 500
- Where they come from. BPC-157 is a 15 amino acid fragment of a protein in human gastric juice. TB-500 is a 7 amino acid fragment of thymosin beta-4.
- By mouth. BPC-157 is stable in gastric juice and worked in rats' drinking water. I found no oral study of TB-500.
- Animal evidence. BPC-157 has dozens of rat studies on gut, tendon, ligament and muscle. For TB-500 itself I found one healing study in rats, described below.
- Human evidence. BPC-157 has three small pilot studies. TB-500 has none that I could find.
TB-500 benefits: what is actually documented
A 2024 paper says it directly: the biological effects of TB-500 have not been documented. The authors then tested it on fibroblasts. The parent TB-500 did not significantly speed wound closure. Only one of its breakdown products did. (Rahaman 2024)
The promising results belong to its relatives:
- The same 7 amino acids without the acetyl group healed skin wounds in aged mice about as well as full thymosin beta-4. (Philp 2003)
- Full thymosin beta-4 and hair. It stimulated hair growth in normal rats and mice by activating stem cells in the follicle. That is the Philp 2004 study above.
- Full thymosin beta-4 on chronic wounds. In two phase 2 trials on stasis and pressure ulcers, it sped healing by almost a month in the patients who did heal. (Treadwell 2012)
- Full thymosin beta-4 eye drops. In 72 people with dry eye, neither main endpoint was met, though some secondary measures improved. (Sosne 2015)
- Full thymosin beta-4 after a heart attack. In a randomized trial of 96 patients, the overall difference in damaged heart area was not significant. Only the subgroup dosed within 8 hours of the procedure did better. (Zhang 2025)
All of those human trials used the full molecule, made to drug standards and given by doctors.
BPC 157 and TB 500 together
One rat study tested the stack. 32 rats had the Achilles tendon cut and repaired, then got BPC-157 at 10 micrograms per kg a day, TB-500 at 60 micrograms per kg a day, both, or nothing, for four weeks. (Biçer 2026)
- Both peptides improved the tissue structure of the tendon.
- TB-500 was the only one that significantly raised the load the tendon could bear.
- The combination did not add anything over either peptide alone.
The authors call it exploratory. Those are rat doses given by researchers, not a protocol.
In people there is one small clinic report on knee pain. 12 patients got BPC-157 alone and 11 improved. 4 got BPC-157 with thymosin beta-4 and 3 improved. No control group, and too few people to compare. (Lee 2021)
TB-500 side effects, and is TB 500 safe?
The honest part. Nobody knows, because nobody has tested TB-500 in people.
- The full molecule looked tolerable short term. In a placebo-controlled study of 40 healthy volunteers, intravenous thymosin beta-4 daily for 14 days caused infrequent, mild or moderate adverse events. (Ruff 2010) A second phase 1 study in 84 healthy volunteers found the same. (Wang 2021) That is the 43 amino acid drug, not the TB-500 fragment.
- Blood vessels and tumors. In mice, melanoma cells made to overproduce thymosin beta-4 grew larger tumors and formed 46.7 lung nodules against 10.9 in controls. (Cha 2003) In 143 people with colorectal cancer, high thymosin beta-4 in the tumor went with worse survival. (Lee 2017) This does not show that taking TB-500 causes cancer. It is the reason reviewers flag anything that drives blood vessel growth.
- No dose is known. A 2026 review for sports doctors says human orthopaedic data for thymosin beta-4 and TB-500 are lacking, and that indications, dosing, frequency and duration remain unknown. Both are banned in sport. (Mayfield 2026)
- Product quality. TB-500 is unapproved and sold through the unregulated market, where a 2026 review lists contamination, impurities and inaccurate dosing among the risks. (Moiz 2026)

What I think
I have never posted a TB-500 protocol, and I am not going to write one here. There is no human dose to report, and it is an injectable.
I did share one case: a man who added BPC-157, GHK-Cu and TB-500 after a year on finasteride and got much better hair density. One man on three compounds tells you nothing about TB-500 alone.
Between the two, BPC-157 has more behind it, and it can be taken by mouth. And my first peptide is still beef gelatin. Start with the BPC-157 guide and the peptide studies.

Is TB 500 safe?
It has not been tested for safety in people. It is unapproved, banned in sport and sold through unregulated sources. High thymosin beta-4 has been linked to tumor spread in mouse and tissue studies, which is why caution is advised.
