Semax Side Effects: What Is Actually Documented

Search for Semax side effects and you get the same list on every site. Nasal irritation, headache, insomnia.

I went to the studies to see where that list comes from. Most of it does not come from the studies.

What the researchers reported

• "None." The team that developed Semax wrote that in no case did it produce negative side effects or complications. That is a claim in a review by its creators, not the result of a safety trial. (Ashmarin 1997)
• Seizure-like EEG activity. In 73 patients with brain damage after oxygen deprivation, the EEG sometimes showed episodes of paroxysmal activity after Semax. The authors advised giving the first dose with EEG monitoring. (Alekseeva 1999)

The second point is the only adverse signal I found in a PubMed abstract. It was in people with serious brain injury, but it is a reason for anyone with epilepsy or a seizure history to stay away.

What the animal studies suggest

• It amplifies stimulants. In rodents, Semax given before amphetamine dramatically increased the dopamine release and the movement caused by the drug. (Eremin 2005)
• It raises serotonin turnover. The same study found a serotonin metabolite in the striatum rising to 180% within 1 to 4 hours.

Nobody has tested Semax together with Adderall, Ritalin, modafinil or an antidepressant in people. The animal result says the combination is not neutral.

What is only anecdotal

• Burning or irritation in the nose.
• Headache.
• Trouble falling asleep when taken late.
• Irritability or feeling wired.
• Hair shedding.

These come from user reports and vendor sites. I did not find any of them counted in a study. That does not make them false. It means nobody has measured how often they happen.

The sleep reports do fit the pharmacology. A peptide that pushes BDNF, serotonin turnover and stimulant effects upward is more likely to keep you awake than to sedate you.

What has never been studied

• Long-term daily use.
• Use in pregnancy or breastfeeding.
• Use in children.
• The modified versions sold online, such as N-acetyl Semax amidate.
• Purity of products sold as research chemicals.

That last one is not theoretical. A European control laboratory has reported Semax in seized, unlabelled preparations. (Vanhee 2020)

The bottom line

Semax looks well tolerated in short courses in small Russian studies. "Looks well tolerated" and "proven safe" are different things, and for Semax only the first is true.
